How State Budget Cuts Impact Continuity of Mental Health Care

Continuity of care between the inpatient and outpatient settings continues to be a challenge. Current hospital payments assume that hospitals are actively involved through discharge and the transition to outpatient settings and advocating for payments for outpatient providers to assist in this process is viewed as duplicative. This undermines mental health care providers’ ability to smoothly transition clients between service settings.

Meeting the credentialing requirements for program services and mental health professionals has posed new challenges. Community behavioral health organizations employ professionals that may not meet private insurers’ credentialing standards (for example, 3 years of post-licensure experience). Community providers have addressed this through contractual arrangements in which quality assurance and supervision requirements substitute for these credentialing standards. Services are billed under a supervisory protocol in which the supervising professional’s national provider identifier is used.

Additionally, some programs offer services that rely on a combination of funding sources such as county, state, and private insurers. In these situations, counties sometimes want to limit private insurance clients’ access to these programs because a portion of the overall program is covered by the county.

Impact of State Budget Cuts on Mental Health Care -

In a dramatic turnabout that may foreshadow dilemmas faced by other states, the governor of Minnesota vetoed funding for the state’s mental healthcare program. The legislature would have extended the program for several months, as a compromise was negotiated to retain elements of coverage for the state’s mental health population – a hospital uncompensated care fund, medication/pharmacy, and “coordinated care delivery systems.” In the system, an accountable hospital-centered program paid a fixed amount to cover about 40% of the state’s mental illnesspopulation who elected to participate. As there is no reimbursement for outpatient clinic and all non-hospital services, providers and consumers now are scrambling to seek disability determination or enroll in Medicare type coverage after the six month state mental illness coverage enrollment period ends.

While these cuts are only effective as of June 1, 2010, it is expected that they will result in increases to the uncompensated care burden on hospitals and community safety net providers.

How Do We Minimize The Impact of Budget Cuts on Mental Health Care?

Many not-for-profit membership organizations representing community mental health and other service provider agencies throughout Minnesota have been working in coalition with national mental health groups on advocacy related to the state’s mental health program changes. Initially, advocacy efforts were focused on encouraging the state legislature to vote in support of expanding the state Medicaid program early to receive additional federal funding (as provided for in the national healthcare reform bill). Unfortunately, this proved to be politically untenable in the immediate future; however, a measure was passed to allow the governor to use executive authority to expandMedicaid coverage for mental illness patients.

While being actively involved in this advocacy process is vitally important to the community behavioral health system, national mental health advocacy medicaid organizations and their members are also evaluating ways in which they can optimize their business practices to meet this changing budgetary reality. Among other strategies, community behavioral health providers are working to develop partnerships with community hospitals to reduce the number of avoidable emergency department admissions and ease the transition from the inpatient to outpatient settings, supporting clients through the disability determinations process so they may become eligible for Medicaid as quickly as possible, and raising funds that will help to cover the cost sharing requirements for state sponsored mental health care and the enrolled clients that are unable to pay.

Through this two-pronged approach that includes both advocacy and pragmatic business considerations, it is hoped that the community behavioral health system will be able to develop new cost-effective ways of delivering services that will be well-positioned to withstand funding changes while taking advantage of new opportunities made available through national and state health care reform initiatives.

Black Men and the Stigma of Seeking Mental Health Help

Neo soul artist, Angie Stone puts to lyrics what many Black women have been thinking about Black men for years when she said “you got mad issues, and you tend to refuse every opportunity to right your wrong”. Because the women in our lives, who know us so intimately and care for us so deeply, can see when we are hurting or responding out of pain. But Black men young and old are walking around with anger, hurt, and stress which is literally sucking the joy out our lives through high blood pressure, impotence, fatigue, alcoholism, and many other physical and emotional manifestations as the result of poor mental health. However, many of us stubbornly refuse to get the mental health help that is needed to be better men – husbands, fathers, boyfriends, or just plain ole emotionally whole.

No, it’s just not “Crazy Uncle Joe” in the backroom of your granny’s house who desperately needs mental health help, but it’s also those brothers out there who seem to have everything together. They are suffering alone, bottling up their emotions, or self medicating. And the ones who depend on them and love them dearly are suffering too.

But why, when mental health help is available?

When I sat down with Dr. Verdi Lethermon (Director of the Psychological Services Division for Houston Police Department and owner of Lethermon & Associates), she said that Black men are reluctant to get mental health help because “Black men have been historically misunderstood, misjudged, and oppressed. Black men have tried to walk a fine line of being assertive and understood without being aggressive. Black men tend not to show their weaknesses because of this. In therapy you have to be vulnerable and let your feelings out and address that.”

And letting feelings out is something that Black men were taught not to do at a very early age. We can all remember what was said to little girls when they fell down and scraped their knees, “aw baby, come here, it’s gon’ be alright.” However, the speech was much different when a little boy fell down and began to cry, “boy stop crying like a little girl, get yo’ a** up and walk it off”. And at those moments, little boys were trained to be grown men who can’t express emotions and are afraid of being vulnerable. But Dr. Lethermon believes that negative emotions have to be dealt with because if not “they eventually come out in an uncontrollable way and then we all pay the prices”.

We know that Black men have been used as scientific and medical guinea pigs from the slave ship to Tuskegee. And while this truth exists, it should not preclude us from getting the mental health help needed. Dr. Lethermon says that when Black men refuse to get mental health help “they tend to be re-victimized by the pain and stress in their lives and continue to victimize others”

So, now may be the right time for Black men to get the mental health help needed! But it takes other Black men and Black people in general to break the stigma about seeing a psychologist and to share mental health challenges. We should take advantage of our oral tradition of storytelling to tell the truth about our mental health. In the same way we ask for a good barber, we should be asking for a good therapist if we really need one.

But how do you know that you need mental health help? Dr. Lethermon says that some of the symptoms are when “you find yourself losing control in areas of your life that you typically have under control, or if a person finds their self slipping into a dark hole and removing self from people, stuff, and routines that you used to do.”

And if you ignore the symptoms, trouble is around the corner. “Anger rises to the surface, because anger is a safe feeling – instead of the real feeling of fear, inadequacy, emotional, stressed, overwhelmed, or powerless – and instead of looking at self that individual blames other people,” Leatherman says.

Fellas, it is not okay to bottle up your feelings or hold back the tears -your issues are real and the way that you feel about them are real also. Jesus wept, Job lamented, David sought counsel, and the O’Jays cried together with their women. So, instead of counseling with brown liquor, with another joint, another woman, or with ESPN – maybe it’s time to call a therapist to talk out these issues.

Ladies can assist in helping the Black men in their lives get the mental health help needed. Dr Leatherman says that mothers, sisters, girlfriends, and wives can “invite him to consider therapy or to a therapist out of your love for him to get him to take care of himself.” Women have great influence on how a man handles his mental health issues – they can be an advocate or a hindrance. Leatherman says that “sometimes women inadvertently stifle a man’s effort to express his feelings by the way women respond to him when he expresses his feelings. Sometimes women minimize his feelings or are overwhelmed and don’t want to hear them, and sometimes women invalidate them. And this can prevent future vulnerability and his willingness to get help, but sometimes they (women) are fighting back. I encourage women to take a look at themselves also – are they hurting or helping the man in their life express his feelings openly.”

So what happens when Black men get the mental health help needed? “It’s pretty amazing”, says Dr. Leatherman, “They feel better about themselves. And that feeling ripples to everybody else – particularly wives and children- they become role models and become the heads that they should be. Then the wives and children are more willing to submit and follow.”

Jill Scott may have said it best when she said “We Need You!” Black women need us, our children need us, and we need ourselves to be whole and emotionally stable. But that cannot happen if we continue to represent false manhood by being afraid to be vulnerable and get the mental health help that some of us so desperately need. Dr. Leatherman says this to Black men, “It takes a lot of courage to come to therapy, everybody won’t do it, but it is one of the best gifts that you can give yourself in life.” And the next generations are counting on you. Our race needs help, so help us God. Future generations are counting on Black men.

The need for behavioral health services in primary care is widely accepted. But the integration of primary care services in behavioral health settings remains controversial despite the fact that individuals with serious mental illness appear to have the worst mortality rates in the public health system. Therefore, mental healthcare organizations are actively pursuing single points of accountability to enhance continuity of care for this underserved population.

- Cost-based-plus financing that supports service excellence: People want and deserve quality services but quality services depend on skilled staff. Low salaries have created – and are perpetuating – a recruitment, retention, and quality crisis for behavioral healthcare. We and those we serve cannot continue to be marginalized. Healthcare reform and economic recovery will depend upon the expansion of information technologies and behavioral health providers must be included.

- Expansion of research-based education and prevention practices: There are mental health and addiction prevention and education programs that work. These include research-based prevention initiatives that reduce the risk of childhood serious emotional disturbance by treating maternal depression, the Nurse-Family Partnership Program that has an array of consistent positive effects across multiple trials, and Mental Health First Aid – an evidence-based mental health literacy program. Now we must adequately fund and support the spread of these interventions to communities across the country.

Here are five facts about the health insurance reform bill and its impact on the treatment of mental health conditions.

With the eventual ban from denying people with pre-existing conditions, those with mental health conditions will have easier access to affordable health insurance. Many times, health insurers refuse to underwrite someone with a diagnosis of depression, anxiety disorder, bipolar disorder, or schizophrenia. That is regardless of how well-controlled the condition may be, and regardless of their physical health. The newly passed bill changes that. Insurers will also no longer be allowed to exclude that particular condition from any coverage they extend.

Small businesses with under 50 employees are now subject to the mental health parity laws, while they were not under the previous bill. Their previous exclusion was due to the increased cost of coverage, but Democrats and other supporters of reform believe that the upcoming exchange markets (run by the states) will assist small employers. The plans sold in the exchanges will meet the parity standards.

The bill doesn’t necessarily mean that people will receive gold-plated mental health coverage. Instead, it states that the coverage should be equal to physical health coverage. If someone picks a bare-bones, high-deductible health insurance plan, their coverage will be limited all-around. What is outlawed is inequality; for example, an insurer cannot charge a higher co-payment for a psychiatrist visit than they would for any other medical specialist. Neither can they deny a patient the opportunity to receive out-of-network mental health treatment, if patients are allowed to seek medical care out-of-network. Also, they cannot limit the number of visits or days of inpatient treatment for substance abuse or mental illness any more strictly than they would for surgery or physical illnesses.

The elimination of lifetime and annual caps on benefits will also be helpful. Mental health treatment can be very costly. Inpatient rehabilitation for substance abuse or eating disorders can cost tens of thousands of dollars per month, which can quickly eat up the limits of most health insurance plans. Fewer patients will suffer from incomplete treatment due to their inability to afford continued help.

The bill’s expansion of Medicaid eligibility will help the mentally ill, whom many experts believe are more likely to be uninsured. Moreover, all Medicaid coverage (not just managed care programs) will now offer equal coverage for mental health conditions.

According to the National Institute for Mental Health, about 26 percent of Americans aged over 18 years are diagnosed with a mental disorder every year. This equates to almost 60 million people. The NIMH has also found that about 6 percent of those, have been diagnosed with a serious mental condition, many of which may suffer from two or more mental conditions. Mental illness, has been found to be a major cause of disability amongst people, aged between 15 and 44 years in the US and Canada.

Of course, mental conditions are not confined to the adult person, as people of all ages and genders can be affected by the disorder. Many children and adolescents are diagnosed with mental health problems, and special care is needed to treat them adequately. Mental conditions that affect children are varied and may be of a temporary nature, however, specialized treatment should be sought to help reduce the chances of the disorder becoming long term. NIMH reports indicate that about 5 percent of children in North America suffer from bouts of depression. This figure increases to around 8 percent for the number of teenagers who are suffering from anxiety and depression.

Many teenagers are also affected by eating disorders, which can aggravate bouts of depression and lead to other problems such as, phobia, inhibition and learning difficulties. Teenage girls who may be suffering from anxiety or depression are susceptible to anorexia nervosa, a very serious eating disorder, which can lead to grave health concerns, even death. Mental conditions, such as these, need to be treated by mental health care professionals who can help young people to adjust to the circumstances that have brought about their disorders.

Attention Deficit Hyperactivity Disorder is a most severe mental health condition that is more likely to affect young boys. Sufferers of ADHD need specialized professional care as well as added personal attention from their parents. A child suffering from ADHD will display little regard for authority and will engage in a range of continuous physical activities much to the detriment of his fellow students, teachers and parents. Treatment, provided by a mental health care professional, for this type of disorder, may include some form of medication.

Early diagnosis of mental health problems, is essential for the successful treatment of the conditions, and the prevention of long term effects that may result in a lasting disability.
